Client Assessments


Objective: Learn how to keep a record of dates and scores for a client's assessments.

 

Accessing Client Assessments

To access Assessments, navigate to the Client Screen. Click Assessments on the side menu.

 

Adding a New Assessment

  1. Click the "Add New" button in the bottom right corner of the screen.  

  2. The Assessment Date will default to today’s date automatically. If you wish to change the date the assessment was performed, click the calendar icon and select the correct date. 

  3. Select the Assessment Type from the drop down menu.

  4. Choose the Assessment Instrument from the drop down menu. 

  5. In the Assessed By field, select the staff who performed the assessment from the drop down menu.

  6. Select the client's assessment Result. Available scoring options will depend on the instrument selected.

  7. If you wish to add a reassess date, click the calendar icon and select the correct date in the Due for Reassessment On field.

  8. If you are satisfied with your entry, click the checkmark icon in the toolbar to save the record.  

    If you wish to close the edit view without saving, click the x in the toolbar to cancel your changes and return to the previous screen.

 

Editing an Assessment

 

  1. Click the assessment record you would like to edit in the assessments list to open the detail view.

  2. Click the edit icon in the toolbar to open the edit view. 

     

  3. If you wish to change the date the assessment was performed, click the calendar icon and select the correct date in the Assessment Date field.

  4. Select the Assessment Type from the drop down menu.

  5. Choose the Assessment Instrument from the drop down menu. 

  6. In the Assessed By field, select the staff who performed the assessment from the drop down menu.

  7. Select the client's assessment Result. Available scoring options will depend on the instrument selected.

  8. If you wish to add a reassess date, click the calendar icon and select the correct date in the Due for Reassessment On field.

  9. If you are satisfied with your changes, click the checkmark icon in the toolbar to save the record.

Options for Assessment Types, Instruments, and Scores can be added or edited in the Organization Management portion of Probatum.
